Add or Change Beneficiary, Successor Annuitant, or Successor Holder
It is important that you notify Outlook if you wish to change the beneficiary on your registered investment plans (RRSPs and RRIFs), Tax-Free Savings Account (TFSA) or First Home Savings Account (FHSA). Your beneficiary information will not be changed automatically by any change of marital status or family situation.

For RRSPs: Adding or Changing a Beneficiary
- Fill out the Designation of Beneficiary Form. If there is insufficient room on the form, please complete two forms, signed and dated the same. Indicate page 1 of 2, and 2 of 2 on the forms.
For RRIFs: Adding or Changing a Beneficiary or Successor Annuitant
Please note: If you are converting your RRSP into a RRIF, be aware that they are considered two separate plans. You must designate a new beneficiary for your RRIF even if you are naming the same person.
Use ONLY ONE of the following two forms:
- If you are ONLY ASSIGNING A BENEFICIARY and NOT a Successor Annuitant for your RRIF, fill out the Designation of Beneficiary Form. If there is insufficient room on the form, please complete two forms, signed and dated the same. Indicate page 1 of 2, and 2 of 2 on the forms.
- If you are appointing a Successor Annuitant and making an (optional) Beneficiary designation, fill out the RRIF Successor Annuitant and Optional Designation of Beneficiary Form.
For TFSAs: Adding or Changing a Beneficiary or Successor Holder
Use ONLY ONE of the following two forms:
- If you are ONLY ASSIGNING A BENEFICIARY and NOT a Successor Holder for your TFSA, fill out the Designation of Beneficiary Form. If there is insufficient room on the form, please complete two forms, signed and dated the same. Indicate page 1 of 2, and 2 of 2 on the forms.
- If you are appointing a Successor Holder and making an (optional) Beneficiary designation, fill out the TFSA Successor Holder and Optional Designation of Beneficiary Form.
For FHSAs: Adding or Changing a Beneficiary or Successor Holder
At this time, the only options available for the FHSA for desgination purposes is Appointment of Successor Holder. provincial Legislation has not yet been implemented to permit a designation of beneficiary to be made on an FHSA. Therefore, a holder may appoint their spouse/common-law partner (CLP) as successor of an FHSA.
Use ONLY Appointment of Successor Holder Form:
- If you are appointing a Successor Holder, fill out the FHSA Successor Appointment Form.

- Your Designation of Beneficiary or FHSA Successor Appointment will be updated within 2 business days of receipt.
