EXISTING MEMBERS

If you already have an FHSA with Outlook Financial, you can simply purchase another GIC or make a contribution to your FHSA savings account.

To open an FHSA Plan

1. Complete the FHSA Application Form.

   Please note that you will require Adobe Reader to complete PDF forms. The PDF Reader is available from Adobe, which is a third party service provider. You will exit the Outlook Financial website and open a new browser tab by clicking on the following link. To proceed, download the free PDF reader here [acrobat.adobe.com].

   Print, sign and date the form, and send the form by mail to the following address:
   
     Outlook Financial

     Winnipeg MB R3C 2G1

2. If you would like to transfer in an existing FHSA from another financial institution, complete a RC721 Transfer from your FHSA to your FHSA form and include it with your completed FHSA application.
3. To invest your funds in a Fixed GIC
   Log in to online banking to verify the FHSA savings account has been opened. Once verified complete and submit our online Deposit Form to provide your investment instructions.

Important Notes:

• To appoint a Successor Holder of your registered plan, please complete an Appointment of Successor Holder form and send in along with your FHSA Application.
• Your FHSA plan will be opened within two business days after receiving your application.
• Within your online banking profile your FHSA will be “view only”. FHSA deposits and withdrawals cannot be made online. Please use the forms available at Manage my Investments to complete any transactions with your FHSA.
