DIRECT TRANSFER-IN AN FHSA

If you have an existing FHSA at another financial institution, you can complete a Direct Transfer-In form to transfer the funds to an FHSA.

IF YOU DON'T ALREADY HAVE AN FHSA PLAN WITH OUTLOOK FINANCIAL

You must first fill out the following:

• Open an FHSA Plan

TO DIRECTLY TRANSFER IN AN FHSA

• To appoint a Successor Holder of your registered plan, please complete an Appointment of Successor Holder form.*
• Your Direct Transfer-In request will be processed and sent to the institution you are transferring from within 4 days. Please be advised that a transfer can take 4-6 weeks.
• Also, rates are subject to change without notice at any time and we do not guarantee rates while funds are being transferred. For more information, please contact us.
• Within your online banking profile your FHSA will be “view only”. FHSA deposits and withdrawals cannot be made online. Please use the forms available at Manage my Investments to complete any transactions with your FHSA.
  
  *Note: the only option available for the FHSA for designation purposes is Appointment of Successor Holder. See ADD OR CHANGE BENEFICIARY, SUCCESSOR ANNUITANT, OR SUCCESSOR HOLDER.

1. Complete the RC721 Transfer from your FHSA to your FHSA form.

   Please note that you will require Adobe Reader to complete PDF forms. The PDF Reader is available from Adobe, which is a third party service provider.    Print, sign and date the form, and send the form by mail to the following address:

2. To invest your funds in a Fixed GIC
   Log in to online banking to verify the transfer has been completed. Once verified, complete and submit our online Deposit Form to provide your investment instructions.
