IF YOU DON'T ALREADY HAVE AN FHSA PLAN WITH OUTLOOK FINANCIAL

You must first fill out the following:

TO DIRECTLY TRANSFER IN AN RRSP TO YOUR FHSA

A direct transfer from an RRSP to an FHSA is subject to FHSA contribution limits, although amounts transferred are not deductible like a FHSA contribution. A transfer from an RRSP to an FHSA does not reinstate an individual’s unused RRSP Deduction Room.

1. Complete the RC720 Transfer from your RRSP to your FHSA form.

   Please note that you will require Adobe Reader to complete PDF forms. The PDF Reader is available from Adobe, which is a third party service provider.    Print, sign and date the form, and send the form by mail to the following address:

2. To invest your funds in a Fixed GIC
   Log in to online banking to verify the transfer has been completed. Once verified, complete and submit our online Deposit Form to provide your investment instructions.
